To renew your license ONLINE please log in to your account - Download Instruction HERE 

If you need a paper renewal click HERE - Please note that paper renewal MUST be postmarked by March 31st, to avoid being returned. It is advised that when sending paper renewals to pay for tracking services to ensure delivery. 

 

To renew your license VIA MAIL: email admin@labmt.org to request a paper renewal form 

Note:  Please verify your CE Certificate contains all information as specified in LAC, Title 46, Part XLIV, Chapter 37, 3703 (C) (2) to avoid application denial.  To avoid a late fee, applications must be approved prior to March 31st.
